Dear Selena,
Can you help me make a code for four labels and a textfield each with a send request button below?
I am currently studying on my own java programming.

 
Name                            Textfield
Street Address                  Textfield
City, Street Address, Zip Code	Textfield
Requested Info                  Textfield
 
Send Request(button)

This is in no way a well-written Java solution, but it should give you the basic idea....
